Information in the signed consent record
The paper form requests your name, US mobile number, consent selection, separate terms acknowledgment, signature, and signing date. The received email or delivery record also provides receipt information. SI records the form and policy version, collection method, receipt date, and any later withdrawal. Do not include credentials, payment information, medical records, or unrelated sensitive details.
How the information is used
SI uses the record to document your choice, confirm owner-only pilot eligibility, administer consent and withdrawal, and provide evidence when required for messaging compliance. The form does not authorize unrelated marketing or sharing your number with other businesses for their use.
Service processing and required compliance disclosures
The public pages are hosted through OpenAI Sites. If you email the signed form, your email provider and SI’s business-email service process that correspondence. Service providers may handle the signed form and related data on SI’s instructions to deliver, store, or protect records. SI may disclose relevant consent evidence to Twilio, communications providers, or authorities when needed for verification, compliance, or a legal requirement. Those limited processing and compliance purposes do not transfer your consent or authorize another sender to message you. Twilio is the selected SMS provider for the proposed pilot; no messages are activated by this page.
No marketing sharing or transfer of consent
Mobile information is not sold, rented, or shared for third-party or affiliate marketing or promotional purposes. The permission to message you is not transferable to another sender.
The following restriction applies to the marketing, promotional, affiliate, and consent-transfer categories described in this section, not to the limited service-processing or required compliance disclosures described above:
All the above categories exclude text messaging originator opt-in data and consent; this information won’t be shared with any third parties.
Retention and withdrawal
Consent records are retained to document authorization and meet applicable obligations. Withdrawal must be recorded and used to suppress further messaging, except any permitted one-time opt-out confirmation. Withdrawal does not necessarily erase evidence needed to document the request or meet legal obligations.
